Carolina Medicare Prime's Physician & Provider Network
Our outstanding benefit package is made affordable to the Medicare population by contracting directly with specific physicians, specialists, hospitals and ancillary providers. When you join Carolina Medicare Prime you will be required to seek non-emergency care at plan "network or contracted" physician offices and hospitals.

At Carolina Care Plan, all medical contracted providers must undergo an extensive credentialing process to ensure that they are qualified and practicing within the scope of their license. As your health plan, we want to ensure that you are receiving quality medical care that is appropriate and in your best interest as a patient. Not all physicians or hospitals are currently contracted.

To assist you in choosing a qualified, contracted physician in our network, we have Carolina Medicare Prime directories for your review.

Availability of Providers

Enrolling in Carolina Medicare Prime does not guarantee services by a particular physician or provider listed in this directory. If you wish to receive care from specific physicians or providers listed, you should be sure the particular physician or provider is accepting additional patients.

Even if a particular physician or provider is participating in the network on the date you enroll, there is no guarantee that the physician or provider will continue to participate during the entire term of your enrollment with Carolina Medicare Prime. These physicians and providers are independent contractors and are not agents or employees of Carolina Care Plan.

Each specialty listing is made at the discretion of the physician and is not a representation by Carolina Care Plan of a particular qualification. {DOI Reg R69-22 Sec VI A.2.}

The physicians and providers listed in this directory provide services for Carolina Medicare Prime. To find out how to use the providers for your coverage, please refer to your Evidence of Coverage. This document explains the terms, conditions, exclusions, and limitations of your health care coverage.
