Carolina PathwaysSM Care Management & Wellness Programs

Care Management Programs

We offer these programs to help members manage the following conditions:

Each of these programs includes:

  • Toll-free access to a Carolina Pathways support team, including specially trained registered nurses, dieticians and educators.
  • Periodic health assessments and access to a Web site designed to promote healthy self-management skills.
  • Educational mailings.
  • Communication with physician.

Carolina Care Plan Disease Management Programs are designed to assist members in the management of their chronic conditions such as diabetes, chronic obstructive pulmonary disease (COPD), asthma, coronary artery disease (CAD) and congestive heart failure (CHF). Members are identified through the Health Risk Assessment survey, medical and pharmacy claims, referrals from physicians, and calls from members.

Working closely with the treating physician, Carolina Care Plan will provide members with education (to include literature and telephonic case management) and resources to obtain their best state of health.

Pregnancy Programs

Baby Steps: This program includes confidential pregnancy screenings, educational materials tailored to a mother's specific needs, and 24-hour telephone access to an experienced obstetrical nurse from program enrollment up to six weeks after delivery.

High Risk Maternity/Neonatal: For members with high-risk pregnancies, this program strives to bring these pregnancies closer to term and decrease neonatal intensive care unit stays. It helps to manage infants with complex needs due to premature birth and/or congenital abnormalities, and includes nurse specialists who can help with risk screenings, coordinating care, and education.

Wellness Programs

Hypertension & Cholesterol Program: This program helps you meet your cholesterol goal and lower the risk of heart disease. You are taught how diet, exercise, and medication prescribed by the physician can help make a positive difference in your health. You will receive a Welcome Kit, newsletters, helpful hint cards that offer practical diet, exercise, and medication compliance tips, and periodic health assessments and surveys.

Hepatitis C Be In Charge Program: This is a free support and educational program that is available if you have recently started on one of the therapies from Schering Oncology-Biotech and CuraScript. You receive comprehensive information through literature and videos, personal contact with nurse counselors and the Website beincharge.com. Nurse counselors will contact you monthly, and are available when you need them.
