Carolina PathwaysSM Care Management & Wellness Programs
The Hepatitis C Be In Charge Program is a free support and educational program for members who have recently started one of the therapies from Schering Oncology-Biotech and CuraScript.
Hepatitis C is a virus that attacks the liver. It is a contagious disease that is spread by contact with human blood and blood products that are infected with the hepatitis C virus. It causes progressive damage to your liver. If it is not treated, it can lead to cirrhosis, liver cancer, and even liver failure. Hepatitis C is now the #1 reason people need liver transplants.1
Program Benefits:
Information and Education: This program helps the patient and physician by supplying the patient with comprehensive information through literature and videos, personal contact with nurse counselors and the Web site beincharge.com. |
Emotional Support: Being diagnosed with a chronic, potentially life-threatening disease provokes various emotions such as confusion or anger. Patients may suffer from emotional side effects of therapy and the emotional stress of whom they may have passed on this disease. The Be In Charge nurse counselors will help discuss emotional issues. When patients have issues managing the side effects, or are just "having a bad day," the Be In Charge nurse counselors are only a phone call away. |
Monthly Calls From Nurse Counselors: Many patients look forward to a call from their personal nurse counselor. This call provides reassurance and positive reinforcement during what may be a difficult time in the patient's life. This call also serves as reminder for various issues while on therapy. |
Confidential: All medical information is kept confidential and is not shared with the employer. |
Also Available: In addition to this program, Carolina Medicare Prime members can find more information online at carolinamedicareprime.com. My Health Zone, an interactive Web site, can be customized to meet members' health and wellness needs, including: personal nutrition, calorie counters, managing weight goals, exercise, and health & wellness information! It also has information regarding asthma, diabetes, allergies, and high cholesterol.
