Carolina PathwaysSM Care Management & Wellness Programs
The hypertension and high cholesterol program strives to help members meet their cholesterol goals and lower the risk of heart disease. This program teaches how diet, exercise and the medication prescribed by the physician can help make a positive difference in the member's health.

All members who complete the 1-year track will receive a diploma and a T-shirt as a way of congratulating them for staying with the program. |
Throughout the year, members will also receive periodic health assessments and surveys, which they are encouraged to fill out and send in to monitor their progress.

Members have access to My Health Zone through the secure Member section of our Web site. My Health Zone, an interactive Web site, can be customized to meet members' health and wellness needs, including: personal nutrition, calorie counters, managing weight goals, exercise, and health and wellness information! It also has information regarding asthma, diabetes, allergies, and high cholesterol.
