Carolina PathwaysSM Care Management & Wellness Programs
We offer two maternity programs for high-risk pregnancies to our members: ROSEBUD® Perinatal and Neonatal Case Management Programs.
ROSEBUD Perinatal Case Management Program: This free program strives to bring high-risk pregnancies closer to term and decrease neonatal intensive care unit stays.
ROSEBUD Perinatal Nurses:

Complete a risk screening to evaluate pregnant mothers at risk for premature delivery or experiencing complications such as diabetes and hypertension. |

Coordinate the most appropriate plan of care, as identified by the health care providers, with services leading to a healthy delivery. |
 |
Provide ongoing education and support of mothers-to-be and families. |
Members will receive helpful information to help make healthy choices during pregnancy from reputable sources such as the American College of Obstetricians and Gynecologists, March of Dimes, and state health departments.
ROSEBUD Neonatal Case Management Program:
Regardless of the advances in medical treatment, some babies will be born prematurely and/or with congenital abnormalities. This free program helps to manage infants with complex needs.
ROSEBUD Neonatal Nurse Specialists:

Work with medical providers to coordinate infant care during hospitalization and assist with discharge plans. |
 |
Provide information on appropriate Neonatal Intensive Care Unit (NICU) levels of care and alternative care sites. |
 |
Educate parents on the infant's diagnosis and care needs to prepare them to provide care at home. |
 |
Coordinate the services and equipment required for care after discharge home. |
 |
Connect parents with available community resources for additional support. |
A Carolina Care Plan Case Manager will refer eligible members to ROSEBUD. A ROSEBUD representative will contact the member to obtain her consent to enroll. Members and physicians can contact ROSEBUD throughout the pregnancy if additional complications develop or if additional services are needed.
